Accessing Your Medical Records

For access to your medical records, we now utilize the services of a medical records company, HealthMark Group.  

To submit your Medical Records Release forms, download records, and pay invoices, get started here:

If you're the patient or guardian and have not yet completed a release form, you can fill out and download a copy of our release form here:

Once the request is fulfilled, you will receive an email that your records are ready for download!  This site also allows you to pay for your records electronically.  If you would like to check the status of your request, you may log into the account that you have created and all of this information will be readily available. 

While accessing your medical record via HealthMark Group is the most secure method of receiving your protected health information, we understand that this may not be right for you.  For alternate request and delivery methods please use:
