Medical Records

Advanced Wellness & Physical Therapy has entered into a formal agreement with the release of information vendor, Chartfast to process medical records requests on our behalf. All requests for medical records can be submitted electronically via the Chartfast website at www.chartfast.com . This web-based system allows you to request records, check status, make payment and retrieve electronic copies of your requested records all in one convenient location!

You can submit your records through our portal at www.chartfast.com following these steps:

1. Click the blue"Request Your Records" in the upper right corner
2. Click register as a new requestor
3. Complete the required fields and click submit (you will then receive an email giving you a temporary password to login)
4. Once logged in, click New Request.
   1. In Custodian/Hospital name select the appropriate healthcare facility
   2. Complete the Request Details fields that pertains to your request
   3. upload the request letter/authorization in the authorization field
5. Click submit

Once you save and submit, a notification is sent to their team to start processing your request. Once completed, you will receive an email to login and retrieve your records. 

While accessing your medical records via Chartfast is the most secure method of receiving your protected health information, we understand that this may not be right for you. For alternate request and delivery methods please use:

For the records request you have submitted prior to your account set up, They enclose an invoice for your convenience. Once paid, these records will be mailed to you. 

To pay your invoice by phone call toll free at 855-764-9430 or mail a check to the address on the enclosed invoice.
